After some time, your 2FA OTP token may go out of sync with the server. This is most commonly seen in HOTP counter-based tokens (including YubiKey HOTPs) when you click the counter several times without entering the OTP into a login prompt. If the counter is incremented too many times, the server will no longer accept the OTP.
To fix this, use the form below. You will need to provide two consecutive OTP codes. This will allow the server to determine the current counter value and resync the token.
